4.1. Personal information such as your full name, date of birth, gender and other such information by which we may identify you.
4.2. Sensitive personal information such as your username and password, financial information such as bank account or credit card or debit card or other payment information, physical, physiological and mental health condition, sexual orientation, medical records and history, biometric information and any other information essential for the provision of care and services to you through Our Platform by our authorised healthcare professionals that the law regards as sensitive personal information.
4.3. Contact information such as your location, residential and/or work addresses, phone number and email address or similar information of your spouse, guardian, family member or of other person/s that you voluntarily provide us.
4.4. Information without which our authorised healthcare professionals may not be able to provide you with adequate or appropriate care and services.

Information such as your history and details of visits to our Platform and its access and usage thereof, information to analyse traffic and usage of our Platform, the Internet Protocol address of your computer or other device used to access our Platform, and where your visit to CareMe’s Platform is through or from another website, then the URL of such website, the web browser used by you for such access to CareMe’s Platform, and the name of your Internet Service Provider.
4.7. Correspondence information such as email or letters you may send us or any correspondence about you sent to us by third parties.

We may use your information, analyse usage patterns and disclose your information within or outside CareMe to support business activities, to maintain and improve care and services provided through our Platform, case management, for quality assessment and improvement purposes of professionals associated with CareMe, to review the care you received through our Platform and to evaluate the performance of our authorised healthcare professionals in caring for you, to identify new services to offer, what services are not needed and whether care and services through our Platform are effective, licensing, conducting or arranging other business related activities. We may remove information that identifies you so that people outside CareMe can study your information without knowing who you are. We may analyse Platform usage patterns for improving product design and utility. We may use and disclose your information for commercial purposes in an aggregated and/or anonymized form for research, statistical analysis and business intelligence purposes.
